    Anatomical features of the ostiomeatal complex and their impact on complications in dental implantation

    The anatomical structure of the ostiomeatal complex (OMC) plays a crucial role in the success of dental implantation procedures, particularly in the maxillary region. This study examines the anatomical variations of the OMC and their impact on postoperative complications in dental implantation. A total of 400 spiral computed tomography (CT) scans of patients aged 18 to 95 years were analyzed to assess key anatomical structures, including the maxillary sinus, concha bullosa, uncinate processes, and sinus ostium dimensions. Findings revealed significant anatomical variability, with concha bullosa observed in 33.2% of cases, uncinate process variations in 31.6%, and a narrowed maxillary sinus ostium in 6.25% of cases. These structural differences were found to influence sinus ventilation and drainage, thereby increasing the risk of postoperative complications such as sinusitis and impaired osseointegration. Statistical analysis highlighted strong correlations between sinus volume, bone thickness, and OMC components, reinforcing the need for thorough preoperative assessment. The study underscores the importance of personalized surgical planning and advanced imaging techniques in mitigating complications and optimizing dental implantation outcomes

    Morphometric thresholds of the ostiomeatal complex and their psychological correlates in sinus ventilation impairment

    The ostiomeatal complex (OMC) is a critical anatomical region ensuring ventilation and drainage of the anterior paranasal sinuses. Variability in its morphology can lead to functional impairment and predispose individuals to chronic rhinosinusitis. Objective: To determine quantitative morphometric thresholds associated with inadequate sinus ventilation based on OMC structure and semilunar hiatus dimensions using CT data and computational modeling. Methods: Morphometric analysis was performed on 400 spiral CT scans. OMC parameters were measured manually and automatically, and 3D models were constructed. Pressure and flow simulations were conducted using the IMED system. Subjects were divided into three groups based on the width of the semilunar hiatus: narrow, moderate, and wide. Parameters such as sinus wall thickness, OMC component density, and estimated ventilation pressure were analyzed. Results: Both excessively narrow (<1.8 mm) and overly wide (>3.5 mm) semilunar hiatuses were associated with impaired ventilation. Optimal airflow occurred in the intermediate range (2.1–3.2 mm). A pressure gradient of <2.5 Pa between the maxillary sinus and nasal cavity was associated with stasis in 73% of cases. Strong correlations were observed between sinus ventilation efficiency and the thickness of the uncinate process (r = –0.75, p < 0.01), as well as medial maxillary sinus wall thickness (r = –0.72, p < 0.05). Conclusion: Functional ventilation of the maxillary sinus requires a semilunar hiatus width between 2.1 and 3.2 mm and a pressure differential above 2.5 Pa. Both morphological extremes are associated with physiological dysfunction, highlighting the importance of patient-specific anatomical assessment in surgical planning

    Anatomy of the maxillary sinus: implications for odontogenic sinusitis development

    This study investigates the anatomical prerequisites that could contribute to the development of this condition. Material and methods: Using multi-slice computed tomography (MSCT), the study examined the structure and relationships of key anatomical features, including the alveolar process, sinus wall thickness, and the position of the teeth in relation to the maxillary sinus. The results revealed that the lower wall of the maxillary sinus is predominantly formed by the alveolar process, with significant variability in wall thickness, especially between the central and lateral regions. In 95% of cases, this wall was closely associated with the roots of the upper teeth, particularly the premolars. A strong negative correlation was observed between the thickness of the alveolar process and the volume of the maxillary sinus, suggesting that larger sinuses tend to have thinner alveolar processes. Additionally, a significant correlation was found between the position of the sinus and the degree of pneumatization, with a more medially positioned orbit in cases of reduced pneumatization.Conclusion: Structural features of the maxillary sinus, such as its proximity to the teeth and its relationship to sinus pneumatization, are critical factors in the development of odontogenic rhinosinusitis. A comprehensive, collaborative approach is necessary for effective management and prevention, with careful consideration of these anatomical aspects during dental and maxillofacial interventions

    CT-based study of anatomical variations in chronic rhinosinusitis patients

    Introduction: Chronic rhinosinusitis (CRS) is frequently associated with anatomical variations that may predispose individuals to impaired sinus drainage and mucosal inflammation. Computed tomography (CT) of the paranasal sinuses provides an essential diagnostic tool for evaluating these variations. Objective: To assess the prevalence and types of anatomical variations in patients with CRS using CT Data. Material and Methods: A retrospective cross-sectional study was conducted on 75 patients diagnosed with CRS. CT scans were evaluated for anatomical variations including concha bullosa, deviated nasal septum, agger nasi cells, Haller cells, Onodi cells, uncinate process variations, and paradoxical middle turbinate. Results: The most frequent variation observed was concha bullosa (64%), followed by deviated nasal septum (61.3%), agger nasi cells (49.3%), and Haller cells (32%). Onodi cells were observed in 21.3% of patients, uncinate process variations in 17.3%, and paradoxical middle turbinate in 12%. Conclusion: Anatomical variations are common in CRS patients and can play a critical role in the pathophysiology of the disease. CT imaging is vital in identifying these variations, which can guide appropriate surgical planning.

    Морфометричні особливості взаємозв’язку коренів зубів із верхньощелепною пазухою

    Актуальність. Анатомічний зв’язок між задніми верхньощелепними зубами та верхньощелепною пазухою має велике клінічне значення у стоматології, отоларингології та щелепно-лицевій хірургії. Мета роботи – дослідити анатомічні взаємовідношення між коренями зубів і верхньощелепною пазухою за допомогою комп’ютерної томографії та оцінити їхнє клінічне значення. Матеріали та методи. Проведено ретроспективний аналіз 120 спіральних сканів комп’ютерної томографії дорослих пацієнтів віком 20–75 років. Вимірювали відстань від верхівок коренів до дна пазухи, товщину кісткової пластинки, ступінь виступання коренів у порожнину та морфометричні параметри пазухи. Результати. Найтісніший зв’язок із дном пазухи виявлено у перших і других молярів, особливо в мезіально-щічного кореня першого моляра. У 12% випадків відсутня суцільна кісткова пластинка між коренями та пазухою. Вікове збільшення пневматизації призводило до зменшення товщини кістки у ділянці дна пазухи, тоді як об’єм пазухи негативно корелював із її товщиною (r = –0,58, p < 0,001). Пряме виступання коренів у пазуху асоціювалося з підвищеним ризиком одонтогенного гаймориту (p < 0,01). Висновки. Взаємозв’язок між коренями задніх зубів і верхньощелепною пазухою характеризується значною індивідуальною варіабельністю. Передопераційна оцінка комп’ютерних томограм є обов’язковою для планування лікування в ендодонтії, імплантології та хірургії пазух з метою зниження ризику ускладнень.

    Аnatomical variability of the ethmoid and sphenoid sinuses

    The ethmoid and sphenoid sinuses exhibit marked anatomical variability that can impact surgical approaches and predispose individuals to complications during sinus and skull base procedures. Objective: To characterize the morphometric patterns and anatomical variants of the ethmoid and sphenoid sinuses based on high-resolution CT data and to analyze their surgical and clinical significance. Methods: A total of 400 CT scans were analyzed. Sinuses were classified by pneumatization type, dimensions, and presence of anatomical variants. 3D reconstructions were performed to assess spatial orientation and proximity to neurovascular structures. Results: Ethmoid sinus complexity was classified into three distinct types, with higher complexity correlating with narrower surgical corridors and increased risk of incomplete ethmoidectomy. Sphenoid pneumatization showed significant variability, including presellar, sellar, and postsellar types. Onodi cells were identified in 13% of cases, closely related to the optic canal. Septated sphenoids were present in 21%, often traversing the internal carotid artery canal. Conclusion: Detailed anatomical understanding of posterior sinus morphology and variation is essential for safe and effective sinus and skull base surgery. This study provides a morphometric atlas to support individualized surgical planning

    Diagnostic management of patients with onychomycoses

    Introduction: Onychomycosis is a fungal infection of the nail, causing discoloration and thickening of the affected nail plate, and is the most common nail infection worldwide. Microscopy and fungal culture are the gold standard techniques for onychomycosis diagnosis. At the same time, the culture method is long-term and requires significant costs. In this regard, a promising direction in laboratory diagnostics of onychomycosis is the detection of genetic markers of onychomycosis pathogens using polymerase chain reaction. The goal of current work is optimization of diagnostic care for patients with onychomycosis through the use of PCR, epiluminescent diagnostics and therapeutic agents that improve the structure of nail plates. Material and methods: The work is based on the results of observations of 343 patients with fungal lesions of the nails of the hands and feet, which were on inpatient and outpatient treatment, as well as data from laboratory and instrumental studies. Mycological examination of patients included microscopic examination of pathological material (pieces of nail fragments) and cultural study. PCR was performed using a reagent kit with primers to determine the presence of DNA specific to Trichophyton rubrum and PCR with panfungal primers. Epiluminescence surface microscopy of affected nail plates was performed. Results: After calculations using standard methods, it was determined that the sensitivity for PCR is 91.9%, for microscopy – 75.9%, for culture – only 44.3%. The specificity for PCR is 71.4%, for microscopy – 83.3%, for culture – 100.0%. The diagnostic accuracy rate for PCR was 90.3%, for microscopy – 76.3%, and for culture – 47.3%. The higher the sensitivity of the method, the more often pathological changes are detected with their help, and accordingly, the more effective it is. The excess of PCR sensitivity rates compared to microscopy by 16.0% and culture by 47.6% and the high specificity rate (71.4%) indicate the possibility of increasing the detection of patients with onychomycosis through the use of the molecular method of research. Conclusions: The proposed algorithm for managing patients and a comprehensive method of treating patients with onychomycosis, involving both antifungal agents and drugs, contributes to obtaining a timely diagnosis and prescribing therapy aimed at improving the structure of the nail plates.

    Sociocultural impact on students in a stressful environment: medical and psychological aspect

    In the context of increasing global academic mobility, international medical students face unique academic, cultural, and linguistic challenges that extend beyond general educational experiences. This study investigates how the language of instruction, sociocultural environment, and institutional context collectively affect learning outcomes, clinical communication skills, and well-being among mobile medical students. Through a comparative analysis of four distinct groups - based on the language of instruction and country type - this study identifies critical linguistic and psychological factors that shape educational success. Results indicate that students enrolled in English-taught programs in non-native English-speaking environments demonstrate high levels of professional comprehension and social adaptation. Conversely, students studying in unrelated national languages report lower integration and increased academic stress. Findings highlight the need for language support, intercultural training, and tailored pedagogical frameworks to ensure academic equity for future healthcare professionals.
