V.; Tkachenko, I. H.Актуальність. На сьогодні не існує достовірних маркерів, які б дозволяли оцінити схильність конкретного пацієнта до розвитку гінгівіту та визначити прогноз захворювання. Дане дослідження спрямоване на оцінку потенціалу кількох однонуклеотидних поліморфізмів відносно аналізу асоціацій з хронічним гінгівітом у пацієнтів з муковісцидозом і без нього. Мета роботи — проведення генотипування дітей із муковісцидозом (з хронічним генералізованим гінгівітом або без нього) (група випадків) та дітей без муковісцидозу (з хронічним генералізованим гінгівітом) (контрольна група) щодо однонуклеотидних поліморфізмів rs1801270 C>A (c.93C>A; Ser31Arg), rs1800896 A>G (-1082 A>G; с.-1082A>G) та rs63750399 (c.2146A>G; p.Val717Ile) у генах CDKN1A, IL-10 та APP відповідно, а також оцінити перспективи їхнього використання в майбутньому для асоціативного аналізу. Матеріали та методи. Сформовано дві групи пацієнтів — дослідна, у яких діагностовано муковісцидоз, та контрольна — без даного захворювання. Генотипування проводили методом полімеразної ланцюгової реакції — поліморфізм довжини рестрикційних фрагментів з детекцією результатів в агарозному гелі. Матеріалом слугували клітини букального епітелію. Для оцінки стану здоров'я ротової порожнини використовували класичні стоматологічні показники. Результати. Пацієнти без та з муковісцидозом відрізняються за ступенем ураження зубів карієсом (F=6,53; p=0,01) та станом гігієни порожнини рота (F=5,56; p=0,02). Фактор генотипу за rs1800896 виявився значущим щодо варіабельності стоматологічних показників пацієнтів з муковісцидозом. Відмінності показників ступеня ураження зубів карієсом, гінгівіту та стану гігієни порожнини рота у контрольній та дослідній групах не залежать від генотипу пацієнта у позиції (-1082) промотора гена IL-10. Висновки. Варіабельність ступеня ураження карієсом зубів та гінгівітом залежить від сумарного впливу факторів «група пацієнтів» та «генотип за rs1800896»: F_DMF=3,97; p=0,03; F_PMA=3,82; p=0,03; F_CPI=4,36; p=0,02. Dirofilariasis of the maxillofacial area is not a widespread condition, its cases are described in short series, therefore, the aim of this work is to share our own experience of managing patients with this rare condition.The work presents our own experience in the management of 14 dirofilariasis patients with lesions of the maxillofacial area, of them there are 3 men and 11 women aged 29 to 54. Most patients sought help in the late autumn, winter and spring periods and only 2 people in the summer period. All patients were examined by a dentist, and an ultrasound examination of the area with a formation was performed. According to the clinical manifestations, the disease began acutely in nine patients and had a picture of inflamed atheroma (2 – in the zygomatic area, 1 – in the parotid, 2 – in the infraorbital area, 2 – in the temporal area, and 2 – in the area of the nasal bridge). All patients were operated on and tumor-like formations in dense membranes were removed, inside which one live nematode 11-18 cm long was found. In 3 observations, fragments of dead, partially lysed nematodes were removed from the oral cavity, the capsules of which imitated a migrating granuloma. According to the location – 5 capsules with dirofilaria were located in the subcutaneous fatty tissue, 3 – in the submucosal layer of the gums fused to the periosteum of the upper jaw, 1 – in the submucous layer of the lower jaw, 2 – in the fibers of the temporalis muscle and 1 – in the fibers of the buccal muscle. In each case, the localization of the nematode was close to the places of accumulation of adipose tissue or large blood vessels. In the postoperative period complex therapy was prescribed, including symptomatic anti-inflammatory and desensitizing agents, and consultation with infectionist was recommended. Thus, according to our observations, dirofilariasis most often affects areas of the face in which there are large vessels and accumulation of fatty tissue. In connection with the prevalence of dirofilariosis in humans and the predominant localization of parasitic infection in the face area, dentists, ophthalmologists and otolaryngologists need to be familiar with this pathology and show definite vigilance. Treatment dirofilariosis patients should be carried out comprehensively, together with parasitologists, with surgical excision of the lesion and appropriate drug therapy.Item type:Документ, Anatomical features of the ostiomeatal complex and their impact on complications in dental implantation(2025) Nazaryan, R.; Sosonna, L.; Iskorostenska, O.; Storozheva, M.; Fomenko, Yu.; Heranin, S.; Ohurtsov, O.; Nikonov, A.; Alekseeva, V.The anatomical structure of the ostiomeatal complex (OMC) plays a crucial role in the success of dental implantation procedures, particularly in the maxillary region. This study examines the anatomical variations of the OMC and their impact on postoperative complications in dental implantation. A total of 400 spiral computed tomography (CT) scans of patients aged 18 to 95 years were analyzed to assess key anatomical structures, including the maxillary sinus, concha bullosa, uncinate processes, and sinus ostium dimensions. Findings revealed significant anatomical variability, with concha bullosa observed in 33.2% of cases, uncinate process variations in 31.6%, and a narrowed maxillary sinus ostium in 6.25% of cases. These structural differences were found to influence sinus ventilation and drainage, thereby increasing the risk of postoperative complications such as sinusitis and impaired osseointegration. Statistical analysis highlighted strong correlations between sinus volume, bone thickness, and OMC components, reinforcing the need for thorough preoperative assessment. Variability in its morphology can lead to functional impairment and predispose individuals to chronic rhinosinusitis. Objective: To determine quantitative morphometric thresholds associated with inadequate sinus ventilation based on OMC structure and semilunar hiatus dimensions using CT data and computational modeling. Methods: Morphometric analysis was performed on 400 spiral CT scans. OMC parameters were measured manually and automatically, and 3D models were constructed. Pressure and flow simulations were conducted using the IMED system. Subjects were divided into three groups based on the width of the semilunar hiatus: narrow, moderate, and wide. Parameters such as sinus wall thickness, OMC component density, and estimated ventilation pressure were analyzed. Results: Both excessively narrow (<1.8 mm) and overly wide (>3.5 mm) semilunar hiatuses were associated with impaired ventilation. Optimal airflow occurred in the intermediate range (2.1–3.2 mm). A pressure gradient of <2.5 Pa between the maxillary sinus and nasal cavity was associated with stasis in 73% of cases. Strong correlations were observed between sinus ventilation efficiency and the thickness of the uncinate process (r = –0.75, p < 0.01), as well as medial maxillary sinus wall thickness (r = –0.72, p < 0.05). Conclusion: Functional ventilation of the maxillary sinus requires a semilunar hiatus width between 2.1 and 3.2 mm and a pressure differential above 2.5 Pa. Both morphological extremes are associated with physiological dysfunction, highlighting the importance of patient-specific anatomical assessment in surgical planning
